1. Admission Procedure
to Panel
Hospital using HealthCare Card
To present HealthCare
Card and NRIC to hospital admission staff before
registration for admission into hospital.
Hospital will authenticate
with Great Eastern to obtain Letter of Guarantee.
Letter of Guarantee
will only be issued after certain validation according
to Policy's terms and conditions.
2. Discharge Procedure
from Panel Hospital
Hospital will authenticate
with Great Eastern on covered amount.
Upon discharge from
Panel Hospital please pay the uncovered charges
before leaving the hospital.
3. Letter of Guarantee
not issued upon authentication by Panel Hospital
Insured Person is
to pay the hospital expenses and submit for reimbursement
claims following the following procedures.
4. Reimbursement procedure
for admission to Panel or Non-Panel Hospital.
All reimbursement claims
with completed documents are to be submitted within
31 days after discharge from the hospital or end
of a benefit disability.
Required documents
All Original Copies
of Bill and Receipts from the Medical Centers
and or
Clinic.
One completed set
of Claim Form and Attending Physician Statement
at
Assured's own expenses.
Other supporting
documents such as laboratory reports, radiology
reports,
ECG and etc.
Xerox copy of IC.
The above documents can
be submitted to the Customer Service Centre or the
Agents' Serice Centre at Menara Great Eastern ,
or to the branch nearest you.
